Thank you for entrusting WellBridge Fort Worth with the care and treatment of your client or patient. Below you will find the steps for making a referral.

Please note, walk-ins and appointments are welcome.

The Inpatient Program at WellBridge Fort Worth

Adults 18 and older experiencing the following may be eligible for admission:

• Suicidal or Homicidal Thoughts
• Depression
• Anxiety
• Paranoia, or Delusional Thoughts
• Hallucinations
• Aggressive Behavior
• Excessive Isolation
• Inappropriate Sexual Behavior
• Sleep Disturbance
• Significant Weight Change
• Poor Concentration
• Requires detox from alcohol, prescription and/or illicit drugs

• Face Sheet
• Nursing/Social Services Notes (last 3-5 days)
• Current Medication List
• Current Labs
• Admission History and Physical
